self-reportedHis AI-optimized Google Ads bring in new patients for about $10 to $20 per booked patient, compared with the most common marketing agency retainer of $1,000 to $5,000 a month before ad spend.
Andrew Yang, MD runs a solo hand surgery practice with 2 virtual assistants, 1 receptionist and 2 medical assistants, and sees 45 to 55 patients a day. Insurers won't negotiate fees with a solo doctor, so overhead is the only lever he controls. Instead of a marketing agency, new patients come from referrals, word of mouth and Google and Facebook ads, and his Google Ads are AI-optimized. He tracks one number: cost per booked patient, not clicks. For admin he uses Dot, Muse and Grok Bot, and says Grok Bot is his favorite.
Who did it
Try it now
- Pull last month's ad spend and count how many new patients actually booked from ads.
- Divide spend by booked patients. That's your cost per booked patient, the only ad number he tracks.
- Turn on Google's AI bidding toward booked appointments (your booking or call conversion), not clicks.
- Check cost per booked patient every month against what an agency retainer would cost you.
- Keep referrals and word of mouth going. Ads are one of several sources.
Here's last month's Google Ads report and our list of new patient bookings. Match bookings to ad clicks or calls where you can, work out our cost per booked patient, and tell me which campaigns to cut or grow. Don't change any campaign without my yes.
Make it run every week
A play works once. Turn it into a routine in three small steps:
- Run it once and check the result yourself.
- Give your AI a schedule. Tell it when to run this play (every week, or every month) and to message you with what it found before you act.
- Keep a log. One line per run: what was checked, anything flagged, what you did.
What to watch out for
Self-reported from his own X article; the $1,000 to $5,000 agency figure is a survey number he cites, not his own past bill. Ads for medical services have extra rules, so keep claims accurate and patient data out of ad tools. Safer version: check the bookings behind the number yourself each month. Build it with Page: a simple new patient page with online booking, so every ad click lands where it can turn into a booked visit.
